When the Practice is closed, please call our usual line and you will be diverted to our Out of Hours Provider or you can just call 111.

WE ARE NO LONGER TAKING PRESCRIPTION REQUESTS OVER THE PHONE  YOU CAN LEAVE THE REQUEST IN THE SCRIPT BOX IN THE LOBBY.

Please remember to allow 48 hours for your prescription requests.

COVID VACCINATIONS- 

If you had your 2nd dose more than 6 months ago you can book an appointment online COVID Vaccination booking link or by calling 119

Eligible patients who need assistance (transport) to go to a vaccination centre should call the local support line on 0115 8834640.  

Patients who are housebound should have now been contacted to arrange a home visit for a vaccination.

CORONAVIRUS- IF YOU ARE WORRIED IN ANYWAY ABOUT THE CORONAVIRUS PLEASE VISIT THE NHS111 CORONAVIRUS SERVICE ONLINE.

Outside normal surgery hours (evenings, weekends and bank holidays) emergency cover for the practice is provided by NEMS Community Benefit Services Ltd Out of Hours service. The Out of Hours service provides medical care for patients that do not need Emergency Department Care. It is a service that is led by a range of clinical staff such as GPs, Nurses, Advanced Nurse Practitioners and Emergency Care Practitioners.

 This service is for problems which cannot wait until the next available surgery. You will be required to provide full information about your problem and will be expected if required to attend NEMS (based on Station Street, Nottingham) if necessary. Advice may also be given if attending for a face to face appointment is not deemed to be required.

 During out of hours you should contact the NHS 111 service if you urgently need medical help or advice but it is not a life threatening situation.

Note: You may want to access your medical records online !!!! (subject to GP approval). All coded entries in a patients record will be visible for patients to view, including consultations, BP readings, referrals, etc.

Please ask at reception for further information.

 


New Patient Registration

Just tell the receptionist that you want to register with this Practice and you will be given registration forms to fill in.

Please provide identification; you will need to provide photo ID and a copy of utility bill for your new address.

Separate registration applications need to be filled in for each member of the family. An appointment will be made with the practice nurse or HCA for new patient health checks/screening.

Change of Personal Details

If you change your name, address or telephone number, please let our receptionists know straight away . If you move outside the practice area (see map above) it will be necessary for you to find a doctor in your new area.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
